I understand that The Playstation will only accept children aged 3 years to 12 years and 11 months old.
For children age 3 years - spaces are limited due to ratio requirements. Please check availability before placing a booking. A space is not guaranteed until confirmation has been given by a member of Management.
I consent to my child(ren) attending The Playstation and understand that the club has policies and procedures and that both myself and my child(ren) agree to abide by them.
I am aware that The Playstation has an obligation to report suspected child abuse/neglect/safeguarding concerns to the appropriate person at Bracknell Forest Borough Council.
Fees should be paid by the date stated on the invoice. I understand that if I am late with payment it will incur a charge and could jeopardise my child attending future sessions.
I understand that collecting my child after 6pm (5:50pm Holiday Club) will result in a charge of £5.00 per five minute block. i.e, 10 minutes will cost me £10. This fee will be due within seven days.
I agree to update mine and my child’s details when they change.
I understand that the information I have given on this form is confidential unless it is in the child’s best interest to share information to professional third parties.
I understand that bookings are non refundable. Bookings can be transferred to another session if 24 hours notice is given. If 24 hours notice is not given, the session will be missed and credits not carried over. Credits will not be issued if a child cannot attend a session, e.g if a child is sick. Credits will not be given if the club has to close unexpectedly through no fault of the The Playstation. For example, closure for bad weather, health and safety issues (no water, no heating etc), deep cleaning due to infection outbreak etc (the list is not limited to just the above). If the club has to close due to a fault of The Playstation, for example due to insufficient staffing, a credit will be offered on this occasion only.
